Notify us of a death

We're very sorry to hear about your loss. We’re here to help you through this process the best way we can.

We know there's a lot to think about when a loved one dies. We want to do everything we can to make things easier for you.

This page will guide you through how you can let us know a policy holder has died. We can then start the claims process so that it’s as smooth as possible. 

It’s important to tell us as soon as you can - so we can make sure we stop sending correspondence and taking policy payments.

Tell us online

Before you begin, it will help us if you have this information handy:

  • Any correspondence or documents from Phoenix Life about the policy
  • The policy number. You’ll find the policy number on letters or documents about the policy.
  • The date of birth and when the person died
  • Country of death. If the person died outside the UK, it’s not possible to notify us online.
  • The last address they lived at
  • We’ll ask for other details from the death certificate, so it’s best to have it with you

Please read through each of these steps to notify us of the death of a Phoenix Life policy holder:

  1. Please refer to the Phoenix Life documentation and locate the address, usually on the top right hand corner of the covering letter.
  2. IMPORTANT - If the previous policy provider was Scottish Mutual, Scottish Temperance, Scottish Provident, Provincial, Prolific Life or PFM, please select the ‘Glasgow’ button below, even if your documentation has a Peterborough address.
  3. Now select which City/Town is shown unless otherwise instructed above.

Please double check before you proceed. It is important that this is answered correctly so that you use the right form for the policy. If the wrong form is used, we will need to contact you and this may delay the processing of your claim.
