Subject Access Request

Phoenix Group (Phoenix) treats all customer information in a confidential and secure manner. For example, we do not sell information to other organisations for direct marketing purposes.

Data protection act 1998

Phoenix Group complies with the Data Protection Act, which lays down a set of standards that must be followed whenever personal information is processed. The Data Protection Act also gives individuals (known as Data Subjects) a number of rights including the right to find out what information, if any, is held about them. This is known as a subject access request.

When do I need to make a Subject Access Request?

A subject access request is a formal request for information for which we charge £10. The formal access request arrangements are not designed to deal with general queries that you may have about your account. We aim to provide you with the information you require without you having to make a subject access request.

What do I have to do if I want to make a Subject Access Request?

If you want to make a subject access request, you must put your request in writing and pay a £10 fee. You can either print off and complete the application form below or send us a letter.

If you put your request in a letter, you should include the following information:

  • Your full name
  • Your date of birth
  • Your current address and previous address
  • It would be useful if you could provide us with a daytime telephone number in case we need to contact you to discuss your request
  • The account number about which you want information
  • Whether your request relates to a specific part of the Company, for example a Head Office department
  • If your request does not relate to an account, please let us know the nature of your relationship with Phoenix and any other relevant information. For example, if you are or have been an employee, you should include details of your staff number

You should send your completed form or letter together with a cheque for £10 payable to “Pearl Group Management Services” to the address on the form.

Please note that if your request relates to a joint account, you should get the other account holders to sign the form or your letter as we cannot provide information about them without their consent.

When will I receive the information?

We will provide you with the information you have requested within 40 days from the date we receive your request and the £10 fee.

Subject Access Request form

The Data Protection Act 1998 defines a data subject as a person about whom data is processed. You can use this form to make a Subject Access Request for information Phoenix processes about you.
