Phoenix Group complies with Data Protection legislation, which lays down a set of standards that must be followed whenever personal information is processed. Data Protection legislation also gives individuals (known as Data Subjects) a number of rights including the right to find out what information, if any, is held about them. This is known as a subject access request.
You can ask for a copy of the information we hold about you at any time. However, if you have a general query about your policy or your information, you do not have to make a formal subject access request – our customer services team can help you.
If you want to make a subject access request, you can print and complete the application form below.
Alternatively, you can contact our Data Protection Officer by emailing us at [email protected] or writing to us at:

If you put your request in a letter, you should include the following information:
- Your full name
- Your date of birth
- Your current address and previous address
- It would be useful if you could provide us with a daytime telephone number in case we need to contact you to discuss your request
- The account number about which you want information
- Whether your request relates to a specific part of the Company, for example a Head Office department
- If your request does not relate to an account, please let us know the nature of your relationship with Phoenix and any other relevant information. For example, if you are or have been an employee, you should include details of your staff number
Please note that if your request relates to a joint account, you should get the other account holders to sign the form or your letter as we cannot provide information about them without their consent.
You can ask for personal information about someone else, provided you have (and provide a copy of) their written authority.
We may contact you if we need further proof of identity for you or anyone else named in your request.
We will provide you with the information you have requested within 1 month from the date we receive your request. If your request is particularly complex, the legislation provides us with an additional 2 months to respond, however we will contact you to explain if this is the case and confirm when we will be able to respond to your request.
You can use this form to make a Subject Access Request for information Phoenix processes about you.
